Appointments:

  1. New patient appointments require a referral from a patient's primary doctor. 
  2. Records pertinent to the patient’s vasculitis history should be mailed to the University of Utah prior to consultation.  These records should include clinic notes, discharge summaries, diagnostic studies, radiographic studies, biopsy reports, and prior treatment regimens. 
  3. The patient’s primary doctor should include a one page summary of the patient's history and treatment regimen.  Any specific questions that either the patient or the patient’s doctor would like for us to address should be included in the one page summary. 
  4. If biopsies have been performed, we request that patients have the reports and the actual glass slides sent prior to consultation.   
  5. Records can be faxed to the attention of Dr. Koening at 801-581-6069.

University of Utah Vasculitis Center Faculty:

Curry L. Koening, M.D., M.S.:  Dr. Koening completed medical school in 2000 at Texas A&M Health Science Center College of Medicine.  He did internal medicine residency and rheumatology fellowship at the University of Utah in Salt Lake City, Utah.  He completed an NIH sponsored vasculitis fellowship through the Vasculitis Clinical Research Consortium (1 U54 RR019497-01) in 2007 at the Cleveland Clinic Center for Vasculitis Care and Research in Cleveland, Ohio.  At the same time, he completed a Master in Science in Clinical Research in the Clinical Research Scholar’s Program at Case Western Reserve University in Cleveland, Ohio.  Dr. Koening brings to the University of Utah expertise in clinical research and in the diagnosis and management of vasculitis. 

Mark Elstad, M.D.:  Dr. Elstad is Professor of Internal Medicine and specializes in pulmonary medicine.  He has extensive experience in managing complicated airway diseases, particularly endobronchial disease and airway stenoses caused by Wegener's granulomatosis.  Both he and Dr. Smith co-direct the University of Utah Airway Disorders Center.  

Marshall Smith, M.D.:  Dr. Smith is Associate Professor of Otolaryngology and Head and Neck Surgery at the University of Utah.  His expertise is in the management of tracheal diseases caused by rheumatic diseases and cancer.  He has particular expertise in managing subglottic stenosis, a common airway disorder afflicting those with Wegener's granulomatosis.  

Brad Katz, M.D., Ph.D.:  Dr. Katz is Assistant Professor in Ophthalmology and Visual Sciences and Neurology at the University of Utah.  His specialty is in neuro-ophthalmology.  Dr. Katz has interests in a wide variety of ophthalmological diseases but as extensive experience in diagnosing and treating giant cell arteritis and in temporal artery biopsies.
